1. How did you learn about the RI Bankruptcy Legal Clinic?
2. Did you make your own appointment through the RI Bankruptcy Court's website or did somebody from the court assist you?
3. Were you able to locate and complete the Agreement for Limited Legal Advice Form and Intake Form before your meeting with the volunteer attorney?
